Ophthalmic Eye Drops: Uses, Formulation, and Bioavailability Challenges
Ophthalmics (eye drops) are sterile liquid preparations designed for direct application to the surface of the eye, usually in very small volumes—typically one or a few drops at a time. They are widely used to treat eye conditions such as dry eye, infections, allergies, inflammation, and glaucoma.
Why eye drops contain saline
Most eye drops include saline (or tonicity-adjusting agents) to make the formulation isotonic, meaning it closely matches the natural salt concentration of tears. This helps improve comfort, reduces irritation, and supports better patient compliance.
Why ophthalmic drug delivery is challenging
Ophthalmic formulations remain a major focus in pharmaceutical technology because drug bioavailability in the eye is often low. After instillation, only a small portion of the drug typically reaches the target tissues due to several natural barriers and defense mechanisms, including:
- Complex eye anatomy that limits drug penetration
- Corneal barriers, including the lipophilic corneal epithelium, which restricts absorption of many drugs
- Limited absorptive surface area of the cornea
- Tear turnover and drainage (nasolacrimal drainage removes the drug quickly)
- Metabolism at the ocular surface, which can break down the drug
- Low conjunctival sac capacity, meaning the eye can hold only a small amount of liquid before overflow and drainage
These factors reduce the drug concentration at the site of action and shorten the contact time of the active ingredient on the eye surface.
